The inspector’s account

On March 26, 2026, the department conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the administrator Gemma Sia Rodriquez. The department explained the purpose of today’s visit. The facility is licensed to operate for (4) ambulatory adults ages 18 through 59. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. The facility is approved for one (1) hospice client.

The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) clients' rooms, two (2) common bathrooms, a staff room, a staff bathroom, a living area, a dining area, a kitchen, a den, an attached garage which house a washer, a dryer and an extra refrigerator and an outside patio area.

Between the hours of 11:44am - 12:15pm, the department toured the physical plant. There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the client's personal belongings was observed. Bed linens, comforters, and bath towels were stocked during the visit. Bathrooms were operational. The water temperature measured (108.1°F & 109.7°F in bathroom 1 & bathroom 2, and kitchen 109.°F).

The department observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to clients. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. Fire extinguisher was charged, and smoke detectors and carbon monoxide were operable. A working landline telephone was available and operable. The last emergency drill was conducted on 03/02/2026.

Between the hours of 10:36am - 11:30am, the department conducted an inspection audit of (4) client records, (5) staff records, and (3) clients Personal & Incidental (P & I) and (4) Client Medication Administration Records which are in complete & in order.
