Facility Evaluation Report
On 11/13/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit. LPA met with Administrator Assistant, Tiffany Loatman and explained the purpose of today’s visit. The facility is licensed for sixteen (16) ambulatory only clients. There are eight (8) clients in attendance today.
Crossroads Community Services is a one-story structure located in a commercial building. The facility consists of Lobby, front entrance with waiting area, (1) directors office, (1) conference room, (2) bathrooms, (1) staff lounge, and (1) activity room. Facility maintains all required posting throughout the facility. LPA checked (5) staff files and (5) client records. The annual fees are not current. LPA discussed with Administrator Assistant.
LPA Shirley and Tiffany walked throughout the facility and all common areas such as the lobby, activity room, and bathrooms were all observed. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water delivered between the required 105 and 120 F. The last fire drill was on 11/3/25.
LPA observed that there is ample seating and space for all clients. The clients bring their own lunches. Facility staff stores snacks and additional food for clients. All walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. There are (3) charged fire extinguishers and all were serviced 10/25. Exterior areas are clean and clear of obstructions and hazards and there are no bodies of water present. The first aid kit was fully stocked and there was a first aid manual available.
No deficiencies were cited during this inspection visit.
An exit interview was conducted, and a copy of this report was provided to Administrator Assistant, Tiffany Loatman.
