The inspector’s account

On 03/19/26 he Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Administrator Eboney Jones as the purpose of today’s visit was explained. The facility is licensed to operate for (4) ambulatory adults ages 18 through 59. The clients are linked to the Harbor Regional Center. The facility is approved for (1) hospice client. Facility fees are current, surety bond is active (PB03138604394), Liability insurance is active (47SPK25845009 exp: 12/21/2026).

The facility is a single-story structure located in a residential neighborhood. It consists of (4) clients' rooms, (2) common bathrooms, an office, a living area, a dining area, a kitchen, laundry area, and rear patio with shaded seating. A working land line was available and operable. There were no bodies of water on the premises. All rooms were inspected, beds and bedding supplies were in excellent condition, adequate lighting is provided, and storage for the client's personal belongings was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature measured between 105-120 degrees F.. Cleaning supplies, toxins, and sharps objects were stored and inaccessible to clients. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. The fire extinguisher is mounted and charged, smoke detectors and carbon monoxide were operable. Fire safety equipment test conducted for March 2026. A Fire Drill was conducted on 03/12/26.

LPA conducted a records review of 4 staff records, 4 client records, 4 medication administration records, and 4 P&I ledgers, no discrepancies observed. Medications were centrally stored and properly locked.

Exit interview conducted, and a copy of this report was provided.
