The inspector’s account

Licensing Program Analyst (LPA) Tena Herrera made an announced visit to the facility regarding updated fire clearance and change of ambulatory status. LPA met with Connie Torres and explained the purpose for the visit. LPA spoke with Administrator Johnette Bellosillo and John De La Cruz and discussed the visit via phone call.

Applicant completed the change of AMB/NON-AMB status and clearance from the fire department was granted on 6/4/25; granted clearance is for (1) ambulatory client in bedroom #1, bedroom #2 & #3 are each approved for (1) non-ambulatory client.

LPA toured the facility and observed the following:

Ramps: Have been added to the front and back entrance as well as to the exits within rooms 2 & 3.

(there were no handrails at any of the exits, exit from the kitchen/dining leading to the back yard has a higher slope compared to the other exits and LPA suggested adding handrails would be a good idea incase a client loses their balance stepping on the slope)

Grab-Bars: Have been added to both restrooms, grab-bars are within the shower and around the toilets.

Doors: Door width within rooms 2 and 3 appear wider, to ensure accessibility for wheelchair and/or walker. Additionally, exit doors have been added to rooms 2 and 3 so clients can exit from their room and into the pathway on the side of the facility.

All entrance/exits, doorways and ramps were unobstructed and had clear walkways.

No deficiencies observed during today's visit, exit interview held and a copy of this report was emailed to both Johnette and John.
