Facility Evaluation Report

Licensing Program Analyst (LPA) Tena Herrera conducted an unannounced case management visit. The purpose of the visit was to verify closure of the facility. LPA met with Administrator/Licensee Taiwo Oyeleye and explained the reason for the visit.

On 12/22/25 LPA received written notice of closure of the facility. The facility was licensed on 11/2/23, LPA has conducted 2 annual visits since then one on 10/31/24 and the other on 11/6/25 during both visits Administrator/Licensee explained that they have had a difficult time obtaining clients, during both visits LPA did not observe any clients at the facility. On 12/22/25 Taiwo Oyeleye provided LPA with written notice stating they wish to cancel/surrender their license.

During today’s visit LPA conducted a tour of the facility, there were no clients observed at the facility, there was no evidence of personal belongings, medication, or files that indicate there were clients at the facility. Administrator/ Licensee Taiwo Oyeleye surrendered the original license to LPA.

Exit interview was conducted and a copy of this report was provided.
