Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tool. LPA arrived unannounced and met with the Assistant Administrator, Archimedes Lopez. The facility is licensed for (3) Developmentally Disabled Adults, ages 18 - 59, of which 3 may be non-ambulatory or bedridden. The home is vendorized by the Harbor Regional Center.

The facility has 3 client bedrooms, 2.5 bath, open living room and kitchen, dining room, and an attached garage. Each client has their own bedroom with appropriate furniture and storage space. The backyard has a covered area. There are extra linens and hygiene supplies. Facility has interconnected smoke and carbon monoxide combo detectors throughout the home. Knives, cleaning solutions, and disinfectants are locked. Food supplies are sufficient with 2 day perishables and a week of non-perishables. The hot water temperature was measured within the required range of 105-120 degrees F. Foods are properly stored in the refrigerator to avoid contamination.

Staff are providing care and supervision to meet the clients' needs and assisting in activities of daily living. There is an awake staff for the overnight shift. Staff are all fingerprint cleared and associated to the facility. LPA reviewed records for 3 staff and 3 Clients. The administrator's (Luis Gonzales) certificate expires on 8/20/25 and the HIV & TB certificate was issued on 4/2/25. Staff have current first aid and CPR certificates. Client files are complete. There is one client with a restricted health plan. Medications are centrally stored. LPA reviewed medications for 3 clients and they are being administered as prescribed. The facility has the updated emergency and disaster plan and conducting disaster drills monthly.

No deficiencies were observed today. An exit interview was held and a copy of this report was given to A. Lopez.
