Facility Evaluation Report

Licensing Program Analyst (LPAs) Galarza and Sakinah Madyun conducted an announced Case Management visit. The purpose of the visit is to follow-up on a facility closure request from Licensee Juanipo Paguio received on April 1, 2025. Licensee allowed entry into the home.

The last resident moved out on April 2, 2025. All four (4) residents were relocated to other Adult Residential Facilities (ARFs).

LPAs conducted a physical plant inspection. The home is vacant and there are no longer any clients residing in the home. Licensee surrendered the license to Community Care Licensing LPAs during the visit. Licensee was informed license is not transferable.

An exit interview was conducted with Juanipo Paguio. A copy of the report was issued.
