Facility Evaluation Report
At 11:30 a.m. on 07/23/25, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with the administrator and disclosed the reason for the visit.
Today’s case management visit was conducted due to an email received by the Woodland Hills-South Adult and Senior Care Office on 06/29/25 from the licensee informing the Department that the licensee wished to close the facility and surrender the license. It was reported on 07/14/25 during an office meeting that all residents had relocated from the facility and the census was zero. LPA toured the facility today at approximately 12:10 p.m. and confirmed that no residents or resident belongings were in the facility. LPA removed the physical license.
No immediate health or safety concerns were observed during today’s visit.
