Complaint Investigation Report

Interviews with staff #1 and staff#2 revealed that staff#2 did not receive training on handling medication as staff#2 dispensed and assisted residents with medication. LPA reviewed staff#2 files and did not find medication training record. Based on review of documents and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Deficiencies are being cited according to California Code of Regulations, Title 22, Division 6.

An exit interview was conducted with administrator. A hard copy of this report and appeal rights were provided to administrator.
