Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted unannounced case management to the facility to follow up on over capacity.

During the Emergency Evacuation Order affecting Altadena facilities, Eaton Fire. This facility was granted a capacity increase waiver to retain a total of seven (7) clients. Due to the client’s original facility burning down to the ground. After the Emergency Evacuation Order was lifted, the Administrator was informed that the approved capacity waiver for seven (7) clients was no longer valid by CCLD.

Although the facility informed the Licensing Office that the seventh client would be moving to another facility due to requiring higher level of care, Licensing visit was conducted to confirm the move.

During this visit LPA spoke with the Administrator, toured the facility and reviewed the clients’ roster. The administrator confirmed the facility census is currently five (5) clients and the seventh client moved out on 06/01/25. The documents did not identify the name of the seventh client.

Based on the information and evidence provided to the LPA the seventh client did not reside or was present at the facility.

No immediate health and safety hazards were noted during this visit.

An exit interview was conducted. Copy of report was issued.
