Facility Evaluation Report
Licensing Program Analyst (LPA) Zabel Chochian arrived at the facility to conduct a required annual visit. LPA met with staff outside the gate. LPA introduced self and reason for visit was stated. LPA was asked to wait for Administrator and was not allowed access to the facility. Program Director Lisa Harris arrived at approximately 10am. The LPA introduced self and explained to the Program Director Lisa Harris that the state visits are unannounced and after verifying identity of the LPA facility staff at the site shall allow LPA access to the facility. Mrs. Harris acknowledged understanding and stated they will ensure compliance in the future visits by the department.
Upon entering facility at approximately 10:30am, the LPA toured the one-story home inside and outside with Program Director to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. Required postings observed posted through-out the facility. Facility is cleared for six (6) ambulatory clients only. Per fire inspection report clients are not permitted in the detached structure /garage located in the back of the property. Attached garage is for storage use only. COMMON AREA: Open floor plan which includes the kitchen, dining area, and a living space area. The common areas were appropriately furnished, and the lighting was adequate. There is a fireplace in the living room which was properly screened. The facility smoke alarm system and the carbon monoxide detector was tested and were operable at the time of the visit. Fire extinguishers observed fully charged and inspected in 10/2/2025. There is an active telephone/line on the premises. Night lights observed in the hallway. Laundry detergent and cleaning supplies are stored inaccessible. THERAPY AND STAFF: Group therapy takes place in the open space area (family room) and individual therapy take place in the designated office/therapy room located near the garage. BEDROOMS/RESTROOMS: The facility has four (4) bedrooms and three in half (3.5) restrooms for clients use. There are two rooms for double occupancy and two rooms designated for single occupancy.
