The inspector’s account

Licensing Program Analyst (LPA) M. Yang conducted an Annual Inspection- Infection Control on this date. LPA met with Administrator Melissa Stockton, Chief Administrative Officer and Amy Soto, Program Manager. LPA discussed the purpose of the visit. LPA completed a tour of the facility with Chief Administrative Officer and Program Manager. 27 clients were present during the inspection.

Upon arrival LPA observed all facility staff wearing facial covering. Visitor log-in/temperature check, masks, and disinfection station were observed upon entry. Facility has one entrance/exit point. Hand sanitizer was readily available for clients and visitors. Social distancing is maintained in the common areas. Hand washing and other various Covid-19 related signs were observed in the common areas. Facility is currently operating in person. The Day Program has four classrooms separated into cohort for activities. LPA observed 30-day PPE supply. Fire extinguisher observed of serviced date: 07/12/22.

Cleaning supplies were observed behind a locked door. Client locked lockers were observed. Client restrooms were tour, observed to be clean, and operational. LPA observed hand washing posting by all sinks. LPA observed locked medications in Program Manager office. A sample of client files were also reviewed to have updated emergency contact information. Staff files have current CPR/ 1st Aid and infection control training.

Exit Interview conducted. The following documents are requested and to be submitted to Fresno CCL by: 11/10/22. The following updated forms were requested: Lic 308, Lic 309 (if applicable), Lic 500, Lic 610D and Lic 9020. A copy of this report was provided to the Chief Administrative Officer.
