Facility Evaluation Report
On 12/10/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual inspection. LPA introduced self, stated the purpose of the visit and met with Administrator Maryann Hagstrom. LPA completed a tour of the facility with Administrator. Clients were observed in the activity room. There were 13 clients were present during visit.
The facility is maintained at a comfortable temperature and no passageway obstructions or fire hazards were observed. Outside was observed clear of debris with adequate outdoor space and seating for clients. Transportation is available and provided for clients. Fire extinguishers throughout the facility were observed with served date: 10/15/25. Kitchen was toured and observed refrigerator at 40 degrees F. Clients bring their own lunch and snacks to the program. Kitchen was toured. Sharps observed stored and locked in kitchen cabinet. LPA observed medications locked in the facility medication room. MAR was reviewed. Medication was checked. LPA observed clients’ personal baskets in the front entrance. Chemicals were observed locked in laundry room.Client bathrooms were tour, observed to be clean, and operational. Hot water temperature was 111.1 degrees F in bathroom 1, 114.5 degrees F in bathroom 2 and 114.7 in bathroom 3. A sample of client and staff files were also reviewed.
Exit Interview conducted. The following documents are requested and to be submitted to Fresno CCL by: 12/16/25. The following updated forms were requested: Lic 308, Lic 500, Lic 610D, and Lic 9020. A copy of this report was provided to the Administrator, whose signature confirms receipt of this report.
