Complaint Investigation Report

Based on interviews and records review, staff that are working in the facility are cleared. LPA reviewed personnel report and staff schedule. All employees were clear to work in the facility. It is unknown if there was a time staff was working in the facility uncleared.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

An exit interview was conducted and a copy of this report was provided.
