Facility Evaluation Report

On 08/22/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the required annual inspection. LPA was greeted by staff, stated the purpose of the visit and was allowed entry into the facility. Administrator arrived to the facility shortly after LPAs arrival. Facility is a Behavioral Management program.

LPAs toured the facility inside and out. Facility was observed to have a kitchen, common area , 5 classrooms, 5 bathrooms. Doors and passageways were observed to be free from obstruction. Fire extinguishers were observed with a service date of 04/10/25. LPA observed 9 staff members present at the facility. Water temperature measured at 109 degrees. First Aid kits were observed to have the required items.

LPA observed the facility to be clean and free from odor. Facility temperature measured at 74 degrees F. Disinfectants/cleaning supplies were observed to be locked in the closet hallway next to the office. Smoke and Carbon Monoxide detector were present in the kitchen. Emergency disaster plan and procedures are in place. Last fire drill was conducted on 5/14/2025. Doors and passageways were observed to be free from obstruction. Client and staff files were reviewed and observed to have the required forms and training.

LPA toured the facility inside and out. LPA observed the required hand washing signs in client restrooms. LPA observed the facility to be clean and free from odor. Emergency disaster plan and procedures are in place. Last fire drill was conducted on 07/14/25.

LPA requested the following updated forms to be faxed to CCLD by: 09/05/25: Designation of Facility Responsibility (LIC308), Personnel Report (LIC 500), Client Roster (LIC 9020), and Emergency Disaster Plan (LIC 610D). No deficiencies cited on today's visit.
