Facility Evaluation Report
On 5/29/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection visit. LPA arrived, stated purpose of visit, and allowed entrance by Direct Support Professional (DSP). Administrator, Jhosan Timbreza contacted by telephone and arrived a short time later to conduct facility inspection with LPA.
Currently, 4 residents in care. All residents were at day program at time inspection began. Residents attend day program Monday through Friday 7:00 AM to 3:00 PM. Facility tour conducted with Administrator. Facility observed to be clean, odor free, and maintained at a comfortable temperature. Facility is 4 bedroom, 3 bathroom with each resident having a private bedroom. Resident bedrooms toured, bedrooms observed to have all required furnishings and lighting to be sufficient. Resident bathrooms toured, water temperature measured at 109 degrees F. Bathrooms observed to have grab bars in the shower and near toilet. Shower also has shower chair and skid resistant mat available. Kitchen toured, facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawer. Medications are centrally stored and locked in kitchen cabinet. All medications observed to have original labels and be administered as prescribed.
Outside of facility toured. Facility has an outdoor shaded seating area available for residents. Exits open free of obstruction.
Fire extinguisher present with a service date of 07/02/2024. Smoke and Carbon Monoxide detectors present and observed operational during inspection. Last Fire drill conducted 4/16/25 according to facility records.
LPA reviewed resident files, staff files, and Disaster Binder. LPA received copies of LIC 500 (Personnel Report) and LIC 9020 (Register of Facility Clients/Residents) during inspection.
No deficiencies were cited during this inspection. Exit interview was conducted and a copy of this report provided for facility records.
