Staff are interfering with residents right to select their own health care agency
On 03/18/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings on the above allegation. LPA introduced self, stated the purpose of the visit, and met with Diana Diaz,Program manager.
During the course of the investigation, the Department conducted interviews, toured the facility, and reviewed records. R1’s physician referred resident for referral and treatment planning for hospice. Based on records reviewed and interviews conducted, there was insufficient evidence to prove that staff interfered with the resident’s rights to select their own health care agency therefore, the above allegation is found to be UNSUBSTANTIATED. An exit interview was conducted. A copy of this report was provided to the Program Manager, whose signature on this form confirms receipt of this report.
