Facility Evaluation Report
Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct the Required Annual Inspection. LPA met with House Manager Cinthia Carrillo. LPA disclosed the purpose of the inspection and was granted entry by Staff Esmeralda Soto. LPA contacted Administrator via telephone who gave permission for House Manager Cinthia Carrillo to sign for this report.
A tour of the facility was conducted with House Manager Cinthia Carrillo. The residence has two A/C units and they were set at 68 F and 74 F. Passageways were free of obstructions inside and outside.
Kitchen toured, supply of food observed and food stored properly for perishable and nonperishable. Medications were stored in a locked Medication cabinet in kitchen. Cleaning supplies were in a locked in cabinet under kitchen sink and in laundry room. Smoke detectors and carbon monoxide detectors were checked and operating. Facility has a pull station fire alarm and a sprinkler system. Fire extinguishers were charged and had service dates of 09/14/23. Fire drill/ earthquake drill completed monthly. Water temperature measured at 120 F.
There was outdoor seating for the residents. No pools or bodies of water.
Resident, medication and staff records were reviewed. Current first aid and CPR were reviewed.
