Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct a case management inspection regarding an incident that occurred on 7/13/23 and 08/04/23. LPA met with Administrator Patrick Howard.
Facility is working on putting a plan in place for C1's behaviors. C1 is currently in the hospital.
