Facility Evaluation Report
At approximately 12:00PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this licensed adult day program unannounced, to conduct an Annual Required inspection. LPA met with Assistant Executive Director's David Beers and Spencer Ridenhour and toured the building. The Day Program was found to be clean and at a comfortable temperature with all exits free from obstruction. LPA observed several main client areas, furnished with tables, chairs, and other cushioned seating. Activity supplies were present. The outdoor area has a covered section to provide protection from the sun. Facility provides several different activities including outings. Clients bring their own lunches to program. Water temperature measured within regulation or was properly marked, at faucets accessible to clients.
At approximately 12:45PM, LPA reviewed 10 of 50 client files which were found to be well organized and contained all the required documentation. Reappraisals were conducted within the last 12 months. Documentation of physician visit, including TB test were on file.
At approximately 1:30PM, LPA reviewed 10 Staff files which contained the required documentation of completed training and first aid/CPR certification. Fire extinguishers were fully charged. Carbon monoxide detector was present.
There were no deficiencies found in the areas inspected.
