Facility Evaluation Report

On 02/03/2026 at 5:00 p.m., Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Case Management visit. LPA met with Medication Technician (Med Tech) Vanessa Ladaban and advised her of the purpose of the visit. Ms. Ladaban notified Administrator Elisabeth Cortes by telephone.

While conducting complaint investigations #15-AS-20260202152031 and #15-AS-20260129093204 on 02/03/2026, LPA conducted a review of facility records. LPA requested the following documents for review: resident admission agreements, Physician’s Reports (LIC 602), Appraisal Needs and Services plans (LIC 625), hospice care plans, home health care service plans, resident care notes, staff schedules, staff roster (LIC 500), staff contact information (telephone numbers), and incident reports for Residents R1 through R8. Facility staff were unable to produce the requested documents at the time of the visit.

Failure to maintain and provide required resident and facility records constitutes a violation of the California Code of Regulations, Title 22. The deficiencies were documented on LIC 809D. Failure to correct the cited deficiencies may result in civil penalties.

An exit interview was conducted. A copy of this report and appeal rights were provided.
