Complaint Investigation Report
This is an amendment to an original LIC9099-C report issued on 11/26/2024.
Allegation: Staff are retaliating against a resident
Based on interviews with R1, R2, R3 and R4 they stated that they have not had any issues with staff/caregivers nor felt that staff was treating them in any kind of retaliated way. R1, R2, R3 and R4 stated that they have not heard of any staff or residents speaking or saying anything of retaliation towards subject resident. S1, S2, S3 and S4 all stated that they are at the facility to do their job, take care of the residents and that they are not doing anything personal against any residents. S1, S2, S3 and S4 all stated that they have not heard any other staff speaking about the subject resident in any negative way.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED .
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
