The inspector’s account

On 04/16/2025 at 11:50 AM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct Proof of Correction (POC) visit. LPA met with Caregiver, Peter and informed the reason for visit. Peter phoned Caregiver, Jenny Lagana to inform. Jenny arrived approximately 30 mins later. Jenny called the Administrator, Tina Camaclang, and left a message.

On 04/09/2025 LPA L. Alexander conducted a Case Management visit in which the following deficiencies were not cleared by the original POC due dates and LPA recited. The new POC due dates were 04/10/2025 (Type A) and 04/15/2025 (Type B) for deficiencies cited.
