Facility Evaluation Report

On 11/06/2025 at 9:00 AM Licensing Program Analyst (LPA) L. Alexander conducted a Case Management visit as a result of licensee requested closure of the facility. Upon arrival at 8:55 AM, LPA met with Licensee, Mihaela Nicula and Administrator, Albertina Camaclang.

On September 19, 2025 Licensee issued a 60 day notice to all four (4) residents and sent LPA a copy via email of notice. LPA obtained notice of R1's, R2's, R3's and R4's placements.

Starting at 9:05 AM, LPA toured entire facility with Licensee and Administrator including kitchen, bathrooms, bedrooms, common areas, upstairs living space, basement, detached garage and backyard. LPA confirmed all residents have moved out. Licensee surrendered her facility license during today's visit.

A forfeiture letter will be mailed to licensee at a later time. Exit interview conducted and a copy of this report provided.
