Facility Evaluation Report
On 01/30/2025 around 02:00 PM, Licensing Program Analyst (LPA) L. Holmes arrived unannounced to conduct an required Annual Inspection, LPA was greeted by one staff upon entry and explained the purpose of the visit. Ingrid Alonso-Rodriguez, Administrative Director Staff #1 (S1) and Ember Avalos, Community Programs Director Staff #2 (S2) were both available to assist.
LPA and S1 toured the facility and LPA observed a visitor sign-in log and sanitizer, and clients with PPE mask to prevent infection. The facility was active with lcients using several stations while socializing. There is a surplus of PPE centrally located inside the facility and Contra Costa County assists with testing. LPA observed hand washing and COVID-19 signs posted. All hand washing stations were equipped with soap, paper towels, and LPA advised S1 to place covered garbage cans in the shared areas. Passage ways were well lit and free of obstruction. Hot water temperature at the shared kitchen sink measured at 110.8 degrees Fahrenheit (F). Four (4) fire extinguishers were last inspected 02/14/2024 and was observed. The Smoke/Carbon Monoxide detectors were observed operational. Emergency disaster drill last completed 12/30/24 Two (2) first aid kits; One (1) first aid kit is complete.
