Facility Evaluation Report

On 9/9/2025 at 10:10am, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct Case Management visit regarding an incident that was reported to CCLD on 9/8/2025. LPA met with Fabiola Dela Torre and Jose Cobian, Program Managers, and explained the reason for the visit.

Incident report for C1 and C2 was sent on 9/8/2025, regarding inappropriate touching in the community. LPA interviewed staff, obtained and reviewed the staff schedule and community planning schedule. LPA interviewed four (4) staff. Two (2) of the four (4) were present at the outing. Both S3 and S4 stated there were two (2) staff and six (6) participants. S3 observed incident and spoke with C1 and C2. Facility will schedule interdisciplinary team meetings for both involved clients. Facility also held a group meeting for men and women after the incident.
