Facility Evaluation Report

On 01/28/206 at 3:35PM, Licensing Program Analyst (LPA) T. Syess-Gibson, conducted an unannounced annual 1-Year required inspection. LPA met with Paulina Sagrero, Staff. LPA explained the purpose of the visit. Armando Valadez, Licensee arrived at 3:49PM. LPA explained the purpose of visit. Administrator currently holds certificate #6070308735 Expires: 10/22/2026. The facility’s fire clearance was approved for two (2) ambulatory and two (2) non-ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the residents. Hot water temperature in the shared clients’ bathroom was measured at 111.9 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. LPA observed a minimum of 7-day non-perishables and 2-day perishables foods. Smoke detectors/carbon monoxide were in operating condition during visit. The emergency disaster plan was last updated 09/25/2025.Fire extinguisher was purchased 06/18/2025. Fire drill was last conducted 12/20/2025. First aid kit was observed to be complete.
