Facility Evaluation Report

On 06/18/20 at 10:00AM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived announced to conduct a case management inspection regarding facility closure. LPA met with licensee, Nicoh Alday. LPA received an email dated 05/15/2026 of Licensee/Administrator advising CCL of forfeiture of license. Facility has no residents at the time of notification of closure. Facility has not had any residents since being licensed.

LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, and outdoor areas. LPA observed that no clients were present during inspection. The Licensee/Administrator provided original license and letter of closure to LPA during visit.

LPA will send a forfeiture letter to the licensee at a later time.
