Facility Evaluation Report

On 04/02/2026 at 01:00 PM, Licensing Program Analysts (LPA) David Doidge conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 03/27/2026. LPA met with Director of Day Services Jason Parks and explained the purpose of the visit.

LPA spoke with Director of Day Services about the incident. S1 was working with C1. S1 was trying to show C1 what kind of behavior S1 did not want to see from C1. Other staff misinterpreted these actions. Director explained that after staff spoke about the incident, it was determined that S1 was not trying to cause any distress to C1. S1 has been training and will be monitored by a supervisor going forward.
