On 8/4/2026 at 9:30AM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct a case management visit to get exemption request denial documentation signed. LPA met with Executive Director (ED), Mary Goldhoff and explained the purpose of the visit.
During visit, LPA verified that person one (P1) is not employed at the facility and that the facility received the exceptional denial for P1 dated 7/17/2026. ED signed confirmation of removal during the visit and noted that P1 was never employed by the facility.
No deficiencies are being cited on this date.
