Facility Evaluation Report
On 2/20/25 at 11:15 am Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to do an annual inspection. LPA meet with Administrator Milagros Quezon and explained the purpose of the visit.
LPA inspected the facility inside out. There is no body of water. Physical plant is consistent with the facility sketch received by Central Application Bureau (CAB) and approved by the fire department. LPA inspected the living room, dining area, kitchen, bedrooms, hallways, bathrooms, side and backyards. Bedrooms were observed appropriately furnished with adequate lighting and drawers. Equipment and supplies for residents' personal hygiene are available and on site.
The following deficiencies were observed:
A person who is living at the facility is not associated
The plastic tablecloth is dirty and covered in food residue
Chemicals cleaner left unlocked in laundry room
Bags of trash and junk are through out the back and side yard
No updated emergency disaster plan posted
A civil penalty of $250.00 will be assessed on today’s date for repeated violation for section number 80087(a)(1) , and a civil penalty of $500.00 will be assessed on today’s date for individual without clearance 80019(e)
Exit interview conducted with Licensee, a copy of this report provided along with an LIC421FC, LIC421BG and appeal rights.
