On 10/30/2023 at 1:55 PM Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to conduct a Case Management. LPA met with Calin Niculescu, Administrator.
LPA was following up on previous case management dated 10/03/2023. The facility was informed that the official cause of death for R1 was covid-19
