04/08/2026 and conducted by Evaluator Jill Clancy-Czuleger

Staff does not ensure facility is free of pests

On 04/15/2026 at 12:00 PM, Licensing Program Analysts (LPAs) J. Clancy-Czuleger arrived unannounced to conduct a complaint visit. LPA explained the purpose of the visit with Administrator Charmaine Collado. During the initial 10-day complaint visit, LPA interviewed staff and residents, collected the pest control service report.

On the allegation:Staff does not ensure facility is free of pests

Based on interviews and observations the facility did have an incident with a rodent being in the building in Febuary of 2026. The facility called Ecoguard (pest control) and had the buliding serviced on March 6th 2026, and continued the service to come out monthly after that.

Although the allegations may have happened or are valid, there are not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.Exit interview conducted and a copy of this report provided.
