On 08/03/2026 at 9:30AM, Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced Health and Safety check due to the department receiving a priority 1 complaint.
Residents in care appear to be safe. LPA was informed that S1 and S2 were working at the facility before a request a transfer of a criminal record clearance . Both S1 and S2 do not have files at the facility.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
