On 06/18/2025 at around 1:30 PM, Licensing Program Analysts (LPAs) L. Alexander and D. Doidge arrived to conduct a Case Management Closure visit and met with Clinical Director, Louise McClellan and Executive Director, James Mercado.
During the visit, LPAs inspected the facility inside and out including but not limited to bathrooms, and common areas. There were no clients observed at the facility.
LPA, L. Alexander, received closure notification from Louise on 05/15/2025 stating that the facility has officially closed with clients. The facility moved locations. Original facility license was surrendered to LPAs during the visit.
