Facility Evaluation Report

On 08/18/2022 at 1:45PM, Licensing Program Analyst (LPA) C. Fowler arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Executive Director, James Mercado and explained the purpose of the visit. Day program operates from Monday - Friday 9:00AM-2:00PM. There were 9 staff observed working with the 13 clients here today.

LPA toured facility with James Mercado and Louise Mcclellan including but not limited to, multiple activity rooms, kitchen, bathrooms, office space. Clients bring their own lunches and snacks. Emergency supplies were observed. Cleaning supplies are locked and inaccessible to clients. Medications are handled/dispensed by this program. There are no bodies of water or fire safety hazards observed. Restrooms are maintained in safe and in sanitary operating condition, and additional equipment for the physically handicapped was observed. Incontinent clients are kept clean and dry, and the facility is free of odors.
