Facility Evaluation Report

On 08/06/2025 at 12:45PM, Licensing Program Analysts (LPAs) Andrew Christy and Ardalan Charchorloo conducted an unannounced annual required inspection. LPAs met with Bernadette Aguila, administrator, and explained the purpose of the visit. Facility is currently at capacity with six (6) residents.

LPAs toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 73.0 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 106.3 degrees Fahrenheit. All bathrooms are safe and in operating condition. There is a minimum of 7-day non-perishables and 2-day perishables foods.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last services on 10/23/2024. Emergency disaster plan last updated 06/18/2025 and fire drills last conducted 07/16/2025. Five (5) staff records and five (5) residents records were reviewed and found to be complete and up to date.
