Facility Evaluation Report
On 03/25/24 at 10:15am Licensing Program Analysts (LPA) Carol Fowler arrived unannounced to do an annual inspection. LPA meet with Program Coordinators Carmen Palacios and Gay Paris Arndt, and explained the purpose of the visit.
LPA inspected the facility inside out. There is no body of water. Physical plant is consistent with the facility sketch. LPA inspected the activity spaces, offices, med rooms, computer rooms, kitchens, bathrooms, and sensory room. There is a comfortable room temperature of 73 degrees Fahrenheit for clients in care. Clients bring their own lunches and snacks but emergency supplies, including water were observed. Centrally stored medications, toxins and are locked and inaccessible to participants. Activity supplies were available. Outdoor activity space were observed furnished with tables, chairs and shade. Incontinent clients are kept clean and dry, and the facility is free of odors . The program has 7 vans used for client outings. The facility has a mitigation plan. Fire extinguishers were observed fully charge and tags showed serviced 05/16/202 Bldg C and 4/23/2024 Bldg D. First aid kit observed and is complete. Fire/Disaster drill was last conducted on 02/05/2025.
LPA reviewed 8 participants records and reviewed 8 staff records and 7 of 7 were fingerprint cleared and associated to the facility.
