Facility Evaluation Report
On 08/27/2025 at 12:30 PM, Licensing Program Analysts (LPAs) J. Clancy-Czuleger arrived unannounced to deliver findings for the above allegations.
While LPA J. Clancy-Czuleger conducted a complaint investigation (15-AS-20250530151456) on, LPA observed that two staff members were not associated to the facility. S1 confirmed that they and S2 were originally working at Elwyn Camino Salano, and started working at Keith home when Camino Salano was temporally closed for construction.
LPA was also informed that the resent administrator hire did not occur. W1 stated that there has not been an administrator associated the home since December 2024.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. $1000 Civil penalties were assessed on this day. Failure to correct the deficiencies may result in additional civil penalties.
