The inspector’s account

On 09/30/2025 at 10:15AM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct a Required 1 Year inspection. LPA met with staff, Jadel Mendoza and explained the purpose of the visit. Administrator Cecilia Hawkins arrived 10:45AM. The facility is currently at capacity with six (6) residents.

LPA toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguishers were observed to be full and last serviced 02/10/2025. One week of nonperishable and 2-day of perishable food supplies were available. Facility temperature is kept at a comfortable temperature of 70.2 degrees Fahrenheit. Hot water temperature in the resident's bathroom was measured at 105.5 degrees Fahrenheit. There was adequate lighting in each room. First Aid kit is complete.

LPA reviewed five (5) resident files and five (5) staff files, and all were found to be complete. Emergency Disaster Plan was last reviewed 09/30/2025. Quarterly emergency drills were last conducted 08/02/2025 . LPA also reviewed client's P&I money with log, and found no outstanding errors.

The following deficiency was found during the inspection:

At 11:15AM, the second floor balcony's barrier is starting to fall apart next to the bench

Exit interview conducted and a copy of this report, along with appeal rights, was provided to the administrator.
