Facility Evaluation Report

On 11/3/2022 at 12:15PM, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct an Infection Control Inspection. LPA telephoned Administrator, Alfredo Limjoco and LPA explained the purpose of the visit. Caregiver, Brenissel Edwards arrived at the facility at 12:30PM. There were not any clients present. The facility is not operating.

Upon entry, LPA's temperature was not checked. LPA did not observe screening station and COVID-19 signs were posted. LPA toured facility including but not limited to common areas, bathrooms stations, bedrooms, kitchen, garage and back yard. LPA observed cough etiquette and physical distancing posted in the common areas. All hand washing stations were equipped with soap and paper towel. Hand washing posters were posted at hand washing stations. Hot water temperature in the shared clients’ bathroom was measured at 104.2 degrees Fahrenheit. Fire extinguisher was purchased on 5/17/2022.

During record review, LPA observed a copy of the Infection Control Plan on file.
