On 10/20/2021 at 01:30PM Licensing Program Analyst (LPA) L. Hall attempted to conduct an unannounced required 1-year inspection. LPA contacted Fredrico Limjoco, Administrator via telephone who stated that the facility is not operating. Administrator attempted to contact other staff to conduct inspection, however, there wasn't any staff available at this time. LPA informed Administrator that LPA will return at a later date to conduct the annual inspection.
