On 5/22/2026 at 2:35pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Sonya Sowell, Lead.
While LPA L. Hall was conducting an announced pre-licenisng/(CHOW) on 5/26/2026. During record review of guardian LPA observed S7 was not associated to the facility.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
