Facility Evaluation Report
On 10/2/2 024 at 4:00pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 09/30/2024. LPA met with Nicolas Dedicatoria and explained the purpose of the visit.
The incident reported occurred on 9/28/2024. The report stated S2 was conducting a count of the medication and observed that two (2) of C1’s medications were not administered during the morning medication pass. S3 stated the medications weren't administered and the facility is conducting training for the staff on 7 rights to administer.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
