Facility Evaluation Report
On 12/9/2025 at 1:20pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding incidents that was reported to CCLD on 11/18/2025 and 1206/2025. LPA met with Nicolas Dedicatoria, Administrator, and explained the purpose of the visit.
One incident occurred on 11/18/2025 at approximately 4:00pm. the incident report stated C1's ATV was to be taken to the tire shop, the staff was unloading the ATV, C1 hopped on the ATV and took off. Staff searched area where C1 would normally go but was unable to locate C1. C1 was found by staff on 11/19/2025 at 9:00am, at the Antioch marina.
The second incident occurred on 12/6/2025 at approximately 6:03am. The incident reported S3 went to the restroom and upon return observed C1's bedroom door was close. S3 checked and C1 was gone. The facility received a call from C1's family member to advise C1 was in the emergency room and needed stitches from a dog bite. C1 had a laceration of the finger and an avulsion fracture of the distal phalanx of the finger. S1 stated the NOC shift is from 11pm to 7am, therefore, there was only one staff assigned to C1.
