Facility Evaluation Report
On 9/30/2025 at 1:50pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 9/24/2025. LPA met with Nicolas Dedicatoria, Administrator, and explained the purpose of the visit.
The incident occurred on 9/24/2025. Incident report stated C1 had ridden ATV for approximately two (2) hours. It was getting dark and staff advised C1 to take his last lap and C1 rode out of the gate. C1 was found on 9/25/2025 at Napa Auto Parts in Pittsburg by the Pittsburg Police Department and taken to Martinez Medical Center. C1 did not have any injuries.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
