Facility Evaluation Report

On 9/9/2025 at 4:00pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 9/7/2025. LPA met with Nicolas Dedicatoria, Administrator, and explained the purpose of the visit.

The incident occurred on 9/7/2025. Incident report stated C1 requested to ride ATV even though it had a flat tire. Staff let C1 take a ride to calm C1 down and C1 took off outside the facility gate. C1 was found on 9/8/2025 at Lone Tree Golf Course and brought back to facility. C1 was taken to the emergency room but was found not to have any injuries. LPA obtained and reviewed draft copy of contract for C1 riding ATV. Contract indicated ATV should be locked at night. S1 stated according to the contract keys should be given to staff once the sun beings to set.

Upon arrival LPA reviewed roster and observed S2 was no associated ot the faciity.
