On 03/01/22 at 4PM, while at the facility for another reason, Licensing Program Analysts (LPAs) Daisy Panlilio and Liridon Fici reviewed a regional center for the east bay (RCEB) incident report dated 02/12/22 which was sent to the RCEB case manager but not to CCLD.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D.
Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties .
