Facility Evaluation Report

On 06/16/2026 at 12:00pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Hernan Hernandez, Direct Care Support. Administrator, Monique Gran, arrived at 12:10pm and LPA explained the purpose of the visit. The administrator currently holds a certificate (#7027713735) that expires on 02/15/2028. The facility’s fire clearance was approved for four (4) ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2) bathrooms. One (1) bedroom used as an office. All indoor passageways are kept free of obstruction. There were no bodies of water observed. A comfortable temperature for clients is maintained at 71 degrees F. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 108.0 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced 08/21/2025. Fire drill last conducted 04/01/2026. The emergency Disaster plan was last reviewed 1/15/2026. First aid kit was observed to be complete.
