Facility Evaluation Report

On 10/7/2025, at 2:50pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Dayanis Matos Tur, House Manager, and explained the reason for the visit.

While LPA L. Hall was conducting a complaint investigation (15-AS-202510031) on 10/7/2025. During record review LPA observed facility did not submit an incident report regarding facility being out of power for eight (8) days. Panel and meter was fixed and power was back on the 8th day.

The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
